Assisted Living Program

The Assisted Living Program at Sedgwick Heights is for medically stable individuals who qualify for nursing home level of care and are eligible for Medicaid.

Individuals in the Assisted Living Program receive all the services in the Adult Home Program, and:

  • Safety checks

  • Escorts to meals
  • Glucose monitoring
  • Assistance with oxygen therapy
  • Bathroom (toileting) assistance
